Unemployment Appeal Letter in Utah

A letter appealing a denial of unemployment benefits. Appeal deadlines are short and strict in every state — file before the deadline shown below, even if you are still gathering documents, and keep claiming benefits each week while the appeal is pending if your state allows it.

What Utah law says

Appealing a denialFile your appeal by the deadline stated in the decision letter. The preferred method is online at jobs.utah.gov/appeals. You can also fax it to 801-526-9242 or mail it to Appeals Unit, P.O. Box 45244, Salt Lake City, UT 84145-0244. A late appeal may be accepted if you show good cause. An administrative law judge holds the hearing by phone.
If you were firedYou are disqualified if you were discharged for just cause, or for a deliberate, willful or wanton act or omission against the employer's rightful interest. The disqualification lasts until you earn at least 6 times your weekly benefit amount in bona fide covered work. If you were fired for dishonesty that is a crime, or for a felony or class A misdemeanor connected with your work, you are disqualified for that week and the next 51 weeks, and those wages are removed from your base period.
If you quitIf you quit without good cause, you are disqualified until you earn at least 6 times your weekly benefit amount in bona fide covered work. You may still be paid if denying benefits would be contrary to equity and good conscience. DWS looks at whether your actions were reasonable and whether you remain genuinely attached to the job market. Quitting to follow a spouse to a new place is not good cause, unless your spouse is in the armed forces and was reassigned for at least 180 days and the other conditions in the law are met.
