For a revocation for points or a major moving violation, apply at the end of the revocation period and attend a virtual reinstatement hearing; alcohol- or drug-related revocations require a written application with proof of a state-certified counseling program. Non-DC-metro residents can mail a reinstatement request with a check or money order to DC DMV Driver Improvement Office, PO Box 90120, Washington, DC 20090. HawaiiA court-imposed driver's license stopper prevents you from getting or renewing a license; it can be imposed for failing to respond to a citation or summons, failing to appear, or not complying with a court order. To clear it, pay in full the amounts ordered in all underlying cases and comply with all court orders (such as a driver education class). A default judgment behind the stopper may be set aside by filing a Motion to Set Aside the Default Judgment within 90 days with an appearance bond equal to the amount ordered; the stopper is lifted when the bond is posted. A traffic abstract shows moving violations and convictions.

IndianaCheck your status free through the Viewable Driver Record or your Official Driver Record (ODR) in myBMV; both list your reinstatement requirements, and the ODR shows the fee for each suspension and a reinstatement fee access code (a certified ODR costs $4 by mail with State Form 53789). Court-ordered suspensions must be resolved with the court, since the BMV accepts only court documentation to close them; insurance suspensions are cleared by proof your insurer submits electronically (Certificate of Compliance or SR22). Pay fees through myBMV, by phone at 888-692-6841 with your access code, by mailing the reinstatement coupon (or Reinstatement Fee Submission Form), or at a BMV Connect kiosk. KentuckyYour suspension notice from the Division of Driver Licensing says whether a fee is required; a $40 reinstatement/re-licensing fee may be required whenever your privilege is suspended. Paying the fee alone does not restore your privileges: all suspension requirements must be met, and paying a fine does not end a suspension for unpaid fines until the reinstatement fee is also paid. Pay online through myDrive (in-state license holders with a linked account and Letter ID; card processing fee applies), at any Driver Licensing Regional office, by phone at 502-564-1257, or by mail with a certified check or money order payable to "Kentucky State Treasurer" to the Division of Driver Licensing, Attn: Central Section, 200 Mero Street, Frankfort, KY 40622. No testing is required for suspensions under one year; vision and written tests are required after suspensions over one year. An active ID card must be canceled before paying the fee.

Box 64886, Baton Rouge, LA 70896, at a public tag agency, or at some field offices by appointment.

MaineThe reinstatement fee is $50, and the total fee for all court-ordered suspensions for nonappearance or nonpayment in effect at one time is $50 (29-A M.R.S. 2486); a juvenile provisional major-offense suspension carries a $200 fee that must be paid at once. Pay online (debit or credit card, $5 processing fee per transaction; the service shows the total fee due), at a BMV branch, by phone or by mail (no fee); you can usually pay in $50 increments. The online service cannot be used to pay court fines or fees. Payment does not mean you are reinstated: call the BMV at 207-624-9000 ext. 52100 to confirm, and do not drive until the BMV confirms you are legal to drive; a letter is also mailed once you are reinstated.

New YorkDMV mails a suspension or revocation order to the address on file, and more information is available through the DMV's "My License, Permit, or ID" online service (my.dmv.ny.gov). A definite suspension ends when the period is over and you pay a suspension termination fee; an indefinite suspension ends only when you complete the required action (for example, answering a ticket, paying a Driver Responsibility Assessment, or clearing child support or tax debts). Suspension termination fees can be paid online (up to 10 a day) with the order number from your letter, at the DMV office listed on the order, or by mail if the order allows. After a revocation, you must apply for DMV approval when the revocation period ends, may need to retake tests, and may owe a license re-application fee or driver civil penalty. Paying a fee does not guarantee restoration if other problems remain on your record.
