What Illinois law requires
Written demand for the rent due stating that unless payment is made within a stated time of not less than 5 days after service the lease will be terminated. So that partial payments do not invalidate it, the notice must prominently state: "Only FULL PAYMENT of the rent demanded in this notice will waive the landlord's right to terminate the lease under this notice, unless the landlord agrees in writing to continue the lease in exchange for receiving partial payment." Service: hand delivery to the tenant; leaving it with a person 13 or older residing on or in possession of the premises; certified or registered mail with return receipt; or, if no one is in actual possession, posting on the premises.

Fill-in draft
Replace every [bracketed] item. If your state or city requires specific wording, a specific form, or a translated version, that requirement controls over this draft. Courts dismiss eviction cases over defective notices — if anything is unclear, talk to a lawyer.
