What Oregon law requires
Written notice. Week-to-week: 10 days. Month-to-month: tenant 30 days; landlord 30 days without cause only during the first year of occupancy. After the first year the landlord may terminate only for tenant cause or a qualifying landlord reason (demolition/conversion, unsafe repairs, landlord/family move-in, sale to an owner-occupant) on 90 days' notice (60 days in a qualifying sale with extra payment); the notice must state the reason and supporting facts, and landlords with an ownership interest in 5 or more residential dwelling units must pay one month's rent relocation at delivery. Service under ORS 90.155 (personal delivery; first-class mail plus 3 days; mail-and-attachment if agreed; email only with addendum and also mailed).

Fill-in draft
Replace every [bracketed] item. If your state or city requires specific wording, a specific form, or a translated version, that requirement controls over this draft. Courts dismiss eviction cases over defective notices — if anything is unclear, talk to a lawyer.
